Insurance is not accepted at this time. An invoice and other information can be provided for you to submit to your health insurance company, if it provides out-of-network reimbursement for Occupational Therapy.
Fees
Please contact provider for an estimate, which varies according to clinic-based or home-based therapy.
Payment Methods
Accepted forms of payment include cash, check, and electronic payment through Zelle and Paypal.
Cancellation/No-Show Policy
If cancellation is necessary, please notify the provider at least 24 hours ahead of the scheduled appointment so that the allotted time may become useable to another patient and to the provider. A cancellation made within 24 hours of a scheduled appointment is subject to a $50.00 cancellation fee. A no-show is when a patient misses an appointment without cancelling, in which case, will result in the full charge of the session rate.
